Registered Nurse
As a Registered Nurse in our palliative care team, you will be the cornerstone of care, providing expert support to patients and their families in their time of need. Job Responsibilities:
• Patient Care: Complete comprehensive assessments, initiate care plans, and provide holistic nursing care focused on the comfort and well-being of the patient.
• Care Coordination: Collaborate with physicians, families, and the entire care team to develop and revise individualized care plans that meet the evolving needs of the patient.
• Education & Counseling: Guide patients and their loved ones through care processes, offering emotional support and practical advice for managing symptoms and improving comfort.
• Case Management: Take ownership of patient care coordination, ensuring all aspects of the treatment plan are effectively carried out.
Qualifications:
• Current RN licensure and at least one year of experience within the last three years in hospice or a similar field.
• Strong assessment and communication skills, with the ability to think critically and adapt care plans as necessary.
• Compassion, empathy, and a true passion for providing palliative care.
• Current CPR certification and a valid driver’s license, with access to reliable transportation.
