Art of Palliative Care is proud to be an Enhanced Care Management (ECM) provider for Medi-Cal members. As an ECM provider, we are committed to helping members with complex needs coordinate their care effectively.

When you qualify for ECM with Art of Palliative Care, you will be assigned a dedicated care team, including a lead care manager who will ensure you receive the best possible care. Here's how our team will support you:

• Comprehensive Coordination: Your lead care manager will communicate with your doctors, mental health providers, specialists, pharmacists, case managers, and social services providers to ensure everyone is aligned and working together to meet your needs.

• Finding Healthcare Providers: Our team will help you identify and choose the right doctors and healthcare professionals for your specific health requirements.

• Scheduling Appointments: Our team will assist you in scheduling appointments for all your health-related services, ensuring you receive timely care.

• Medication Management: Our team will help manage your medications, ensuring you take the right medications at the right times and avoid any potential issues.

• Transportation Assistance: If you need a ride to your doctor or pharmacy appointments, our team will arrange transportation for you.

• Community-Based Services: Our team will help you find and apply for essential community-based services, such as housing support or access to medically nutritious food, to enhance your overall well-being.

• Post-Hospitalization Support: After any hospital visits, our team will ensure you have the necessary follow-up care, coordinating any additional services or appointments required for your recovery.
