At Art of Palliative Care, we support individuals with complex medical and social needs through a coordinated, whole-person approach. Our services integrate Enhanced Care Management (ECM), Justice-Involved (JI) program support, and palliative care to ensure individuals receive consistent, accessible, and patient-centered care.

We understand that navigating healthcare systems, managing chronic conditions, and transitioning between care settings can be overwhelming. Our role is to bridge these gaps by providing structured coordination, clinical support, and access to essential services.

Face barriers related to housing, transportation, or access to care

Are justice-involved individuals transitioning back into the community

Our programs are designed to support individuals at higher risk of care disruption and help them achieve stability and continuity in their care.

OUR APPROACH

Whole-Person, Coordinated Care

Our approach focuses on addressing both medical and non-medical factors that impact health. We integrate:

Medical care coordination
Behavioral health support
Social determinants of health (SDOH)
Community-based services

This ensures individuals receive comprehensive, continuous, and patient-centered care across all stages of their health journey.

OUR ROLE

Supporting, Not Replacing Care

Art of Palliative Care works alongside existing healthcare providers and systems.

We do not replace primary or specialty care. Instead, we:

  • Coordinate and organize care across providers

  • Support individuals in navigating complex systems

  • Ensure continuity during transitions

  • Provide an added layer of clinical and supportive care

Our goal is to make care more connected, accessible, and effective.
